Our medical team is dedicated to providing the highest academic standards with the convenience of community-based treatment. Though you will be embraced by our warm, homey atmosphere, your treatment plan will be backed with the most advanced treatments that clinical research has to offer as well as the latest technology.

As we have very little staff turnover, our patients enjoy a continuity of care that is unmatched elsewhere. We always strive to be in tune with patient needs.

  • If this is your first visit to our practice, welcome. We are devoted to providing the most comprehensive medical care in the most comfortable environment possible.
  • If you are a new patient, you will be asked to complete a set of forms that will provide us with the information we need to give you our best care. This packet will include information regarding your medical history, a medical release form, and any medications you are taking, and a patient information sheet. It is important that you bring your insurance, and prescription cards with you.
  • Please sign in at the front desk every time you visit the clinic. Even if your visit is for lab, injections or chemotherapy, signing in at the Front Desk will let us know that you have arrived.
  • If your physician is unavailable, we have a Nurse Practitioner on site to assess and evaluate your condition.
